Lesson Seventy-nine Synchronous ventricuIar pacing with direct capture of the atri-oventricuIar conduction system:FunctionaI anatomy,terminoIogy,and chaIIenges
Ideal cardiac stimulation with pacing would involve direct and sole capture of the proximal conduction system without ventricular or atrial myocardial sensing or capture, not affect valvular function, and overcome distal bundle-branch and Purkinje pathology that results in bundle branch block or electromechanical uncoupling. In this review, the functional anatomy of the conduction system, the existing nomenclature with regard to specific sites of stimulation, the advantages and disadvantages associated with each possible approach toward ideal conduction system pacing are discussed.
