小剂量干扰素治疗慢性HCV感染者的疗效及不良反应评估

摘要：

目的:观察小剂量干扰素联合利巴韦林治疗慢性丙型肝炎病毒(Hepatitis C virus,HCV)感染者的疗效及不良反应.方珐:针对各种原因不能耐受标准治疗方案的慢性HCV感染者,给予个体化的小剂量干扰素联合利巴韦林治疗,观察患者的抗病毒疗效及不良反应.结果:小剂量干扰素治疗组93.9％(62/66)的患者可耐受48周以上的治疗,治疗结束时病毒学应答(ETVR)者占83.8％(52/62),持续病毒学应答(SVR)者占53.8％(28/52),复发者占26.9％(14/52),而对照组均未获得病毒学应答,且60％(12/20)的患者病毒量有轻微升高的趋势；治疗组丙氨酸氨基转移酶(ALT)和门冬氨酸氨基转移酶(AST)持续正常患者比例较对照组显著升高[95.3％(41/43)vs 20.0％(4/20)；95.2％(40/42) vs10.0％(2/20),P＜0.05]；而治疗组白蛋白下降、门脉增宽患者比例较对照组显著降低[1.6％(1/62)vs35％(7/20)；3.2％(2/62)vs 30％(6/20),P＜0.05]；治疗组中无肝癌发生及明确的肝病进展,而对照组新增肿瘤患者3例,肝病进展患者7例,两组比较差异具有统计学意义(P＜0.05).结论:小剂量干扰素联合利巴韦林治疗HCV感染者的耐受性好,不良反应少,与未抗病毒的对照组比,可提高对病毒和肝脏炎症的控制,延缓疾病的进展.
